• DDS or DMD degree from a CODA-accredited program

• National Board Dental Examination, Part I (Official copies of scores must be emailed to: postdds@dentistry.ucla.edu)

• Official NBDE Part I scores

• Official NDBE Part II must be submitted by June 1st prior to matriculation

• INBDE scores (if taken instead of NBDE part I/II) must be submitted

• 3 letters of recommendation

• Recent passport-sized photograph (2" x 2")

• $100 non-refundable application fee – PAY HERE

*Verification of U.S. citizenship or U.S. permanent residency must be received by June 1st of the admission year or acceptance into the program may be withdrawn. Failure to provide proof of U.S. citizenship or U.S. permanent residency after the start of the program shall result in automatic suspension and may result in dismissal from the program.
