New Patients and Payment Information

STEP #1:

If you are interested in becoming a new patient, please choose from the following options:

–> Option A: Please call (310) 392-4103, you will then be given access to complete a medical history form electronically and other information related to dental care at UCLA. If you have questions, you can also email the clinic at vdc@dentistry.ucla.edu

–> Option B:  Download the New Patient Registration packet, fill out and email to VDC@dentistry.ucla.edu, send via fax to (310) 392-8513, or bring with you to your first appointment.

STEP #2:

Before becoming a patient of record at the UCLA Venice Dental Center, you will first be screened to determine if your treatment needs are a good fit for the teaching environment. A full series of X-rays or basic X-rays will be taken as needed. The fee for the full series of X-rays is $54.

STEP #3:

Once the screening process is complete and the patient is accepted as a potential teaching case, he/she will be scheduled for a comprehensive oral examination. The fee for the oral examination in the student clinic is $49 and patients will receive a full-mouth treatment plan created by an assigned faculty and pre-doctoral student. Once the patient signs the treatment plan, he/she becomes a patient of record and then procedures will be started in the following appointment.

If your treatment needs are complex, you may be assigned to the Venice Residency Clinic (Venice AEGD) and a secondary screening may be required. The fee for the oral examination in the Venice AEGD clinic is $67. If your treatment needs are more complex than can be treated in Venice AEGD, we will determine if your dental needs can be addressed by more specialized care at:
