Admissions and Applying

• Proof of your status as a U.S. citizen or permanent U.S. resident*

• DDS or DMD degree from a CODA-accredited program

• Successful completion of a CODA-accredited postgraduate training program in prosthodontics or its equivalent

• National Board Dental Examination, Parts I and II (Official copies of scores must be emailed to: postdds@dentistry.ucla.edu)

Applicants must submit the Maxillofacial Prosthetics Application directly to:

The following materials must also be sent to the Postgraduate Programs Officer:

• Curriculum vitae

• Personal statement

• Official NBDE Parts I and II scores (Official copies must be emailed to postdds@dentistry.ucla.edu)

• Official undergraduate, dental school, and postgraduate transcripts

• 3 letters of recommendation

• Optional: Recent passport-sized photograph (2" x 2")

Incomplete files will not be considered.

*Verification of U.S. citizenship or U.S. permanent residency must be received by June 1st of the admission year or acceptance into the program may be withdrawn. Failure to provide proof of U.S. citizenship or U.S. permanent residency after the start of the program shall result in automatic suspension and may result in dismissal from the program.
