New Patient Information

If you are looking to become a patient of our clinic, all new patients must attend an initial screening appointment to be accepted. Please note, due to high interest and lack of providers in the community for people with special needs and medical complexities, we have a waitlist of 24-36 months for the initial screening appointment.

Please fill out our New Patient Request Form or visit tinyurl.com/uclaspcnew to be added to our waitlist.
If you have questions about your waitlist status, contact Social Work at 310-206-6670.

Need to Make, Cancel or Reschedule an Appointment?

If you are currently a patient of record with our clinic, and have questions regarding appointments, billing, or your care, please contact (310) 825-6510.

Social Work at SPCC

The Department of Social Welfare and SPCC at UCLA are collaborating to re-imagine patient-centered care. Our goal is to meet patients where they are and to increase social support as individuals navigate the healthcare system.

We offer the support of a Licensed Clinical Social Worker (LCSW) and Social Work Graduate Student(s) at SPCC during the academic year. Services are available most Mondays and Wednesdays.
Social work can be contacted at (310) 206-6670 or email:
Steph Tuazon, LCSW: sntuazon@dentistry.ucla.edu
